Instructions:

Print two copies; fill out the top half of the contract through the credit card and payment authorization line.  A current credit card must be on file to maintain an active rental contract, regardless of how payment is made. 

Level I instruments require a $50 deposit and are rented at $25 per month, plus tax.

Level II instruments require a minimum deposit of $100 and are rented at $45 per month, plus tax.  We reserve the right to increase the amount of the deposit if necessary.  Instrument rental policies are shown at our web page Ellsworth Music: Retail Division.

Call Ellsworth Music to reserve the instrument you wish to rent.  You may pick it up at either location.
